JF&CS’ Aviv Older Adult Services offers case management services, financial assistance, education and support groups at various locations for caregivers, facilitated by licensed clinical social workers and professional counselors.
We often see clients who are juggling career, family needs and caregiving at the same time, while others live too far from their parents or aging loved ones to help them meet their daily needs. We know that when clients have trouble understanding how to help the elders they care about, it’s difficult to make sense of the options. Geriatric Care Management (GCM) provides assistance for you and your loved one in finding resources, making decisions, and managing stress. This important service provides “one stop shopping” for care for you and your loved one.
JF&CS’ Aviv Older Adult Services provides referrals to services you need in the community, such as assisted living, adult day care and home care, as well as other resources.
The Kosher Food Pantry provides food and/or grocery gift cards for any community member struggling to meet basic needs and they are then better able to direct their limited resources towards other necessities such as housing, medicine and utilities. While we help all people we want our Jewish clients to feel confident that pantry items meet kasruth standards.
JHLC’s membership-based community health and wellness organization offers innovative programs and services to help older adults to continue living independently in their own homes and provides neighborhood seniors with the tools they need to “age in place.”
Services to Older Refugees focuses on providing education about and access to existing services in the community to refugees 60 years of age and older. JF&CS’ Aviv Older Adult Services works with senior citizen organizations to help them better serve refugees. The program also provides culturally appropriate activates to help combat loneliness and isolation.
JHLC’s private home care and supplemental staffing agency offers one-on-one medical and non-medical care in your own surroundings by trained and compassionate nurses (RNs, LPNs) and nurse assistants (CNAs) at a moment’s notice, 24 hours a day, 7 days a week.
The Toco Hills NORC program is designed to help keep older adults safe in their homes and more fully engaged with one another and with their community. Our programs and services provide older adults with access to home safety improvements and enrichment opportunities that provide a sense of empowerment, and remove barriers to participation such as transportation, affordability, and lack of knowledge. In addition, the NORC helps increase the local community’s capacity to help support older adults who wish to age in place. Our NORC program provides a broad range of services that match the varied needs and interests of the older adults who live in the Toco Hills community and surrounding areas in DeKalb County.
